Agreement and Consent

I understand that in British Columbia a Criminal Record Check is legally required for anyone working with the Vulnerable Sector (children, youth and vulnerable adults). I therefore agree to complete a CRC and have it returned to NSAC as soon as possible. I further grant NSAC permission to perform an internet search on me and to review and consider any information found posted by me on the Internet.

I understand that if North Shore Alliance Church approves my volunteer application and later determines, in its discretion, at any time that I am not suitable for volunteer service in the church or for the volunteer position for which I am applying, the church may terminate my volunteer service or position for any reason without advance notice.

I understand that North Shore Alliance Church is responsible for the welfare of any person or persons entrusted to my care, and thus I will cooperate fully with the staff in fulfillment of my duties and will keep all information I encounter in my role as a volunteer, confidential. If at any time I determine that for any reason I am unable to support or adhere to or follow the policies, procedures or doctrine of North Shore Alliance Church, I will inform the church and will gracefully and quietly resign my volunteer position. If my supervisors find that I am in conflict with any of the policies, procedures or doctrines and we are not able to resolve the issue, I will gracefully and quietly agree to resign my volunteer position.

I give my authorization and consent to NSAC to utilize my personal information provided above, in ways relevant to the purpose of my submission, such as but not limited to: entering it into the NSAC electronic database, adding it to applicable NSAC email list(s), etc. (Note: Should you desire to be removed from the email list(s) in the future, just click on the ‘Unsubscribe’ link provided at the end of NSAC emails, or contact the church office for assistance).
