Please use this form to register your preteen(s) in grades 5, 6, & 7 for the 2026/27 year. We're stoked to have them join us!
Completion of this form is required every year for participation in any and all NSA Preteens events (including Sunday mornings, Friday night events, off-site events, campouts and retreats, etc.)
If this form is not completed in its entirety, participation may not be permitted. This form is super important for the safety of all our preteens, volunteer leaders, and staff!
If you have more than one preteen, be sure to include each individual's information and clearly state which is which.
**Only a preteen's parent/guardian can fill out this registration form on their behalf. If a preteen's parent/guardian is unable to fill out the form, please contact Pastor Josh ASAP to get it sorted.**
NOTE: By filling in this form you're allowing us to email you ministry updates about upcoming events and what topics we are covering during our Sunday teaching times.
